Improves recovery when work is interrupted: an integration that was cut off between writing its files and recording the result is now reconciled exactly once when the app restarts, instead of being replayed or lost, and work created from an out-of-date checkout is refused rather than written onto a newer tree. Fixes a failed turn being shown as if it had succeeded - a turn that fails now seals as a failure with a cross instead of a green check, and a tool call that fails carries the cross while it runs. Active mode now clears a stale model-reported hold when a fresh authoritative goal becomes active, while leaving pauses, account limits and other stops untouched.
